CCP supervisor will be responsible to oversee the contracts and activities related to participant services. They function under the direction of the Agency manager.

Qualifications:

An individual with training and experience in home care industry or related company.

Bachelor’s degree in human services, social work or related field.

Combination of skills and experience that indicate the participant has ability to perform the supervisory activities.

Proficient in Microsoft Office Products (Outlook, Word, PowerPoint, Excel, etc.)

Strong interpersonal, verbal and written skills to complete client documentation.

Responsibilities:

Activities of homecare supervisor shall include but are not limited to:

  • Documenting participant contacts from private pay, veteran affairs, CCU, MCO and activities related to participant services in the participant’s file.
  • Preparing, reviewing reports and service calendars.
  • Monitoring receipt procedures in the conduct of essential shopping and errands as stated in the person-centered plan of care.
  • Providing input to the care coordinator on the services that are needed for each participant as a result of conferences with the homecare aides or in-home visits.
  • Planning, preparing and documenting contact and quarterly conferences with each assigned homecare aide.
  • Evaluating each assigned homecare aide quarterly and annually.
  • Coordinating the homecare aide’s activities with other components of the person-centered plan of care as required.
  • Making and documenting 90 days in-home supervisory visits to a participant’s home for each assigned homecare aide.
  • Maintain client confidentiality per HIPAA and Agency policy guidelines.
  • Follow emergency procedures in the event of any medical or non-medical situations.
  • Follow client rights at all times.
  • Report any incidents or client changes immediately to the manager.
  • Making home visits, as necessary, to provide hands-on training and assistance; and
  • Initiating and/or participating in participant staffing discussions with case manager, as necessary.
  • Explain the EVV procedure of clocking in and clocking out for their work schedule during hiring process. Also, creates EVV ID for both homecare aides and participants.
  • Detailed explanation of in-home care services do’s and don’ts of caregiving.
  • Prepare a schedule of Homecare Aide for the participant being served in the Electronic Visit Verification (EVV) system
  • Monitor daily and weekly the Electronic Visit Verification (EVV) system to make sure the Homecare Aide are following their programmed schedule for each participant assigned to them
  • Make sure that there are memos written in the EVV system for any schedule/service deviation by confirming with the participant and homecare aide
  • Be professional and courteous with team, clients, workers, CCUs, including clear communication, responsibility, and appearance.
  • Answers high volume of incoming calls and questions on discussion portal promptly, courteously, and in a caring friendly manner, always ensures to provide good customer service.

Home care Aide is an unlicensed employee, trained to provide home care services for clients under the direct supervision of the supervisor. Homecare aide provides services include dietary management, household chores, grooming/dressing assistance essential to the client’s health and wellbeing, and provision of and instruction in reporting of signs, symptoms, and/or changes in the client’s condition. The homecare aide is assigned to a client by the coordinator or supervisor and is given a written plan of care.

Qualifications:

An individual with training and experience in home care or related company.

A minimum of one (1) year experience in a working environment.

Completion of an initial training and must have a criminal background check.

Must be able to hear and speak in a manner understood by most people.

Current CPR certification preferred.

Responsibilities:

The duties consist of a combination of activities, which include, but are not limited to:

  • Preparation of nutritious and appetizing meals.

Completion of an initial training and must have a criminal background check.

Responsibilities:

The duties consist of a combination of activities, which include, but are not limited to:

  • Perform tasks assigned by the supervisor.
  • Provide personal care services which may include assistance with bathing, getting in and out of bed, teeth, mouth, denture and hair care, assistance with mobility and ambulation, changing of bed and laundering the bed linens and personal clothing, skin care, care of glasses and hearing aids, assistance with dressing and undressing, toileting, including use and care of bedpan, urinal, commode, toilet, light cleaning in essential areas of the home during personal care activities, meal prep, food purchasing, meal serving, simple transfers, including bed to chair or wheelchair and reverse, other delegated tasks according to the client’s service plan of care.
  • Confer as required with the supervisor regarding client’s progress.
  • Promptly reporting any significant changes observed or reported in the client’s condition to the supervisor. In addition, notify supervisor of any safety issues.
  • Follow universal precautions/proper infection control when providing client service.
